60 minutes - $120
90 minutes - $180
Full payment is due at time of service. I accept cash, check, credit/debit, health savings and flexible spending cards.
Cancellation / Rescheduling / LATE ARRIVALS
Cancellations: Cancellations require 24 hours notice, If you are unable to cancel or reschedule within 24 hours you may be charged the full appointment amount upon next visit. Please contact Anita Koch, LMT for cancellation and rescheduling assistance.
No show: No-show appointments will be charged the full amount for the scheduled appointment upon your next visit.
Late arrival: Arriving more than 15 minutes late to appointments may shorten the session time to accommodate appointments scheduled after yours. Please call to let me know you are coming for the remainder of time scheduled.
Illness: I do not charge for a cancellation or reschedule appointment if you or a dependent are sick or experiencing a medical emergency.
Note: All appointments cancelled or rescheduled without 24 hours notice are reviewed on an individual basis by Anita Koch, LMT
Fee for service / out of network provider
I am not contracted with any insurance companies and do not offer insurance billing. Please contact your insurance provider to obtain Out-of Network Provider coverage information.
A paper invoice will be provided upon request if you choose to submit for a reimbursement. Invoice requests made prior to your scheduled appointment date and time will be available in paper form upon arrival. Invoice requests on or after your scheduled appointment can be sent by USPS mail or available upon your next visit.
I do not accept email, text or fax submission of confidential or private personal information. Please inform me of any changes to your client records to ensure I have accurate updated information.
